MAIB Chiefton Accident Investigation Report

The loss of the 'Chiefton' on August 12 last year was a very serious marine tragedy. It was one of the most serious accidents on the Thames in recent years and one which caused the death of the tug's engineer / deckhand Darren Lacey, a respected and popular member of the river community.

The Marine Accident Investigation Branch (MAIB) report into the accident is comprehensive and thorough.

Following the accident, the PLA has reviewed the practices and procedures for the conduct of such tows and has already taken actions as detailed in the MAIB report.

The PLA continues to extend its sympathy to Darren Lacey’s family.
